    In complex fitness to practise (FtP) cases, defense arguments often struggle to objectively separate a pharmacist’s individual cognitive error from the ambient, systemic pressures of the pharmacy environment (such as commercial structures or staffing strains).
    I am exploring the development of a structured forensic tool—a clinical governance liability matrix—designed to help expert witnesses and defense solicitors systematically map out these layered systemic failures. The goal is to provide an objective framework that identifies when an error was driven by organizational vulnerability rather than individual negligence.
    I would love to gauge interest from defense solicitors, expert witnesses, and superintendents:
    Do you find current legal defense frameworks adequate at isolating ambient/systemic pressures from individual culpability?
    Would an objective forensic matrix like this be a valuable asset in your case preparation or governance audits?
