Date of Decision: December 21, 2020

Pharmacist Suspended Again Following Continued Lack of Insight Into Dishonesty, Substance Misuse, and Unlawful Possession of Medicines

Date of Decision: September 9, 2020

Allegations: Found in possession of unlabelled prescription-only medications (Letrozole, Zomorph, Bendroflumethiazide, Aciclovir) at home without authorisation. Pharmacist Given 18-Month Conditional Registration for Mismanagement of Controlled Drugs and Pharmacy Standards

Date of Decision: October 26, 2020

Allegations: Failed to maintain accurate and timely entries in the Controlled Drugs (CD) registers, including for methadone, Zomorph, Sevredol, Medikinet, and others. Allowed poor storage conditions, including unhygienic premises, out-of-date CDs, and inaccurate CD balances. Did not carry out regular balance checks or controlled drug audits, despite multiple inspections and advice from NHS England and the GPhC. Pharmacist Removed from Register Following Conviction for Theft and Forgery Involving Controlled Drugs

Date of Decision: April 10, 2025

Allegations: Convicted of theft and forgery relating to prescription medications while employed as a Senior Clinical Pharmacist. Created and submitted false prescriptions—both handwritten and electronic—using forged signatures and falsified patient records. Accessed patient personal data to select vulnerable patients and hide criminal activity.
