Pharmacy technician receives GPhC warning over chemotherapy ordering dishonesty and expired stock records

Date of Decision: September 30, 2026

Registrant's Role: Pharmacy technician

Allegations:

  • The registrant omitted to order a patient’s chemotherapy medication, Abraxane.
  • When the medication did not arrive, the registrant told a pharmacist and nursing staff that she had ordered it before 27 February 2024, knowing that statement was untrue.
  • The registrant subsequently failed to acknowledge that dishonesty on several occasions.
  • Between January and June 2024, the registrant incorrectly processed expired high-cost pharmacy stock. Instead of booking it out to the EXP2 cost centre as waste, the stock was booked against named patients who had not been prescribed those medicines. The notice explains that this would allow the trust to recoup the cost from the NHS rather than bear the cost itself.

Outcome: Warning

GPhC Standards Breached:

  • Standard 6 – Pharmacy professionals must behave in a professional manner.
  • Standard 8 – Pharmacy professionals must speak up when they have concerns or when things go wrong.
