- Attended work on 12 March 2017 as the Responsible Pharmacist while unfit and/or under the influence of cocaine.
- Admitted to two colleagues on separate occasions on the same day that he had consumed cocaine.
- Used cocaine on one or more occasions between January 2017 and 12 March 2017, other than the day mentioned in the first allegation.
- Failed to cooperate with the GPhCโs investigation between 27 October 2017 and 16 January 2018 by refusing to undergo a drug test.
Outcome: Suspension from the register for 8 months, with an immediate interim suspension order.
